63 year old 5'6”, 124 lbs who had an initial breast augmentation surgery 37 years ago through a periareolar approach. She had complaints of bilateral breast capsular contracture with resulting deformity. Intraoperatively we discovered bilateral silicone implant ruptures. Therefore, bilateral capsulectomy was performed with placement of permanent sutures to close the medial pocket. She desired to keep her current bra size of 36C, therefore 450 cc Natrelle Inspira Soft Touch Smooth Round High Profile Silicone Implants were placed. She was also started on Accolate to help decrease the chance of recurrent capsular contracture. Bra size 34D.

43 year old 5'3”, 150 lbs who had prior breast augmentation with full mastopexy. She had placement of 485 cc Natrelle Inspira Soft Touch Silicone High Profile implants and resulted in bottoming out (implant malposition). She underwent bilateral capsulorrhaphy with permanent suture to reset her inframammary fold. Bra size remained at 36D.

67 year old 5'6”, 130 lbs who had her initial surgery 30 years ago with placement of 300cc textured McGhan saline implants. She was not happy with the look and complaints of firmness. She also desired to go smaller. Therefore she underwent bilateral breast revision with removal of breast implants, bilateral breast capsulectomy and placement of Natrelle Inspira Soft Touch Silicone implants, SSLP 220cc (low plus profile) on the right and 165cc on the left. She was also started prophylactically on Accolate to help decrease her chances of developing capsular contracture again. Bra size remained at 36C.

61 year old 5’4”, 127 lbs with history of 30 year old silicone implants that demonstrated rupture on MRI desired replacement of silicone implants and mastopexy.  Her original implants were placed in front of the muscle (subglandular).  She underwent a staged procedure, which included bilateral capsulectomy and removal of ruptured silicone implants as well as full mastopexy.  She was allowed to heal and then underwent subsequent breast augmentation with placement of 385 cc smooth round full profile silicone implant, placed behind the muscle.  Her bra size changed from 36DD to 36D.
